The following information is required for all loss run requests; 
    a. Policy Holders Name 
    b. list all Policy Number(s) needed

If you are not the agent for the account, we will not process a loss run request without an agent letter of record letter signed by the insured on their letterhead. (Privacy Act) 
We try to process all requested within 48 hrs.
